Our Mission

To provide exemplary consultation services for persons who wish to remain in their home as they age

Initially, we have a conversation about your goals. Secondly a home assessment will be completed. During this we will perform a detailed assessment and check list including:

Our primary goal is to help you “Love where you live”, prevent falls, and maintain independence. Initially we have a conversation with you about your goals over the phone or via zoom. During this time we will establish a time to meet you to complete an on site and thorough assessment of your home. We will complete and supply an individualized checklist which is not a generic checklist for “one size fits all”. You will retain a copy of this checklist. As occupational therapists with CAPS training we have a combined experience of over 50 years in home safety and functional assessments. We are able to see the person, how they function in their environment, and how the environment can be modified for optimal safety and fall reduction. 

Often, the recommendations made are simple and include moving furniture, removing trip hazards, and improving lighting. In cases where more in depth and comprehensive modifications are needed  Forever Home Modifications will facilitate a conversation with the home owner, family members, and needed professionals to complete this option. We can be involved as much, or as little as the client desires to ensure a complete and safe final product. 

Our home safety assessments are not considered a medical or healthcare service, occupational or physical therapy even though our services might be provided by a licensed healthcare provider. As such, our services are not covered benefits under Medicare or any other federal or commercial health plan.
